3/12/2024 EnchantaVaughn Approved Behavior: At the beginning of the session, client expressed how she feels overwhelmed at times. Client wants to be able to balance everything and still be a kid. Client is afraid that she will be held liable for anything that goes wrong since sheâs the oldest sibling. Client expresses that she misses having the attention to herself and wish she had her own room to do more girl stuff. Client was not happy with the way her mom expressed her recent behaviors and became very silent and distant at which mother reports these behaviors last for about 3 days and 10 minutes in duration. Behavior: Client was apologetic for the way that she responded to what her mother was saying about her past behaviors and stated she wanted to fix any problem she had. Client shared that she will work on building a better relationship with her mother, if her mom would let her do more for herself even if that means it takes her longer to do so. Mother shared that she allows client to do more than she should be allowed to do, especially at her age. Client disagreed at these statements and proceeded to ask for more space in the home at which these behaviors last up to 2 minutes in duration when she cannot have her way. Intervention: QMHP commended client on utilizing creative strategies to increase her understanding of feelings and emotions. QMHP processed client feelings and offered choices of what to do when faced with conflict or challenged in the home. QMHP will assist with reducing anger-provoking situations through self-talk and role play scenarios in order to prevent escalation and dealing with anger assertively. QMHP will continue to assist client with managing anger and frustration by reviewing and role-playing effective coping skills. Intervention: QMHP practiced calming techniques with client that can be used in moments of high stress and anxiety. QMHP educated client about using good manners at home and offering alternative ways to model appropriate language that client should use to ask for things or to get someoneâs attention. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support and understanding of rules and expectations. QMHP will monitor effects of coping practices daily and report progress. Response: Client communicated that she felt safe and comfortable talking to the in-home counselor. Both client and guardian provided feedback, actively participated, and engaged with QMHP. Client was slightly hyperactive and required one on one attention to reduce symptoms. Client was provided with redirection for displaying emotional distress to minor irritants. Client practiced some of the examples of alternate behaviors (e.g. praying, drawing a picture, saying affirmations, and repeating positive âI-statementsâ). Response: Client was receptive and open to the interventions, as she was attentive and focused on the information and activities that were being processed without any difficulties. Client demonstrates problem solving by staying calm and using her senses. Client displays good social behavior, as observed and supported by QMHP. Client is reported building healthy relationships in the home. Mom enjoyed seeing client do the right thing for once. Mom and client promised to practice techniques and review and complete the in-home assignment.
